To begin, verify if your insurance covers weight loss surgery by checking their website or requesting a copy of their bariatric policy for necessary approvals. If your insurance does not cover the procedure, we provide financing options, including Care Credit, to assist patients who may not meet criteria or have exclusions in their plan.

Insurance Requirements

Psychological Evaluation

All insurance plans mandate a psychological evaluation as part of their requirements. We can offer you two recommended options for this evaluation.

6-Month Diet

Some medical insurance plans may require you to complete a 6-month dietary and exercise plan before surgery. You can document this by seeing your primary care doctor or Dr. Kurian.

Nutritional Assessment

All insurance plans require a nutritional assessment. You can contact us to speak with our nutritionist, Meg Galligan, for further details.

5-Year History

Several insurance providers mandate that applicants provide a comprehensive five-year weight history as part of their coverage requirements.

Recommendation Letter

Most insurances typically require a recommendation letter from your primary care provider (PCP) endorsing bariatric surgery.
