If your insurance does not cover the procedure, we provide financing options, including Care Credit, to assist patients who may not meet criteria or have exclusions in their plan.

Psychological Evaluation

All insurance plans mandate a psychological evaluation as part of their requirements. We can offer you two recommended options for this evaluation.

6-Month Diet

Some medical insurance plans may require you to complete a 6-month dietary and exercise plan before surgery. You can document this by seeing your primary care doctor or Dr. Kurian.

Nutritional Assessment

5-Year History

Several insurance providers mandate that applicants provide a comprehensive five-year weight history as part of their coverage requirements.

Recommendation Letter
